Vaccine Registration Link for Thursday June, 17^th – Tompkins County Health Department

The Tompkins County Health Department will be conducting a COVID-19 vaccine clinic at the Groton High School Gymnasium (400 Peru Road) on Thursday, June 17^th.  It will be a Pfizer clinic and anyone 12 years old or older is eligible.  Anyone under 18 must have consent from a parent or guardian.

Attached here is a one-page Pfizer fact sheet, the Pfizer Emergency Use Authorization Fact Sheet, and a consent form.  The consent form can be printed and brought to the clinic or there will be forms onsite to be signed.

Individuals without computer and/or internet access may also call 2-1-1 (877-211-8667) during regular business hours 8:30am-5:00pm and someone can register them over the phone.
